check www.astbelgium.be I bought mine there... price +/- 100€ (which is not cheap but) they last 3 to 4 years (if you don't lose them focourse 
).
They'll put some liquid in your ear and you'll have to sit still for about 10 minutes to let it turn hard.
In the end it's a good investment because once you've got eardamage you can't repair it anymore.
You can also get them at laperre hear-aid shops.

It's not a model actually, the model is made inside your ear. The only thing there is to decide is the material they use. They'll ask you what kind of sound you want to keep out (e.g. motorcycle sounds, factory noise, bass, trebble, everything,...) and they'll suggest a material.
